Director of Operations – Post-Acute Care

Position Summary:

The Director of Operations for Post-Acute Care plays a pivotal leadership role in overseeing the operational performance of multiple facilities across Maryland. This position is responsible for ensuring the delivery of high-quality, efficient, and patient-centered care by aligning operational excellence with organizational strategy. The Director will serve as a liaison between client facilities and corporate functions while driving improvements in provider satisfaction, patient outcomes, and organizational growth.

The ideal candidate is a clinician, (Physician Assistant, Nurse Practitioner or Registered Nurse) with leadership experience in post-acute care and a strong understanding of care coordination, quality metrics, and value-based care models.

Key Responsibilities

    Operational Management
  • Oversee daily operations of multiple post-acute care facilities, ensuring efficiency and compliance with organizational standards.
  • Act as a liaison between facilities and corporate teams, including Payroll, Credentialing, Human Resources, Provider Enrollment, Revenue Cycle,Finance and Recruitment.
  • Recommend and implement process improvements to drive operational and administrative efficiencies.
  • Monitor key performance indicators (KPIs) and implement strategies to achieve operational goals.
    Clinical & Operational Leadership
  • Provide oversight of post-acute programs including SNF, home health, hospice, and rehab partnerships.
  • Ensure compliance with state, federal, and accreditation standards.
  • Develop and implement clinical protocols to improve care transitions and reduce readmissions.
  • Monitor quality outcomes, patient satisfaction, and performance metrics.
  • Serve as the clinical liaison between acute and post-acute settings.
    Clinical Interview Responsibility
  • Conduct clinical interviews for open job requisitions, assessing candidates for clinical competency, scope-of-practice alignment, patient care standards, and cultural fit.
  • Partner with hiring managers to evaluate technical skills, decision-making ability, and adherence to evidence-based practice.
  • Provide structured feedback and hiring recommendations to ensure selection of high-quality clinical talent.
    Financial & Performance Management
  • Collaborate with the CCO on financial reporting and accountability for each region.
  • Contribute to the development of annual budgets and manage contracts to meet financial targets.
  • Use KPIs and data-driven insights to identify strategies that maximize profitability.
  • Support Revenue Cycle Management (RCM) to ensure processes yield positive financial outcomes.
    Customer Engagement & Client Relations
  • Maintain and strengthen relationships with Skilled Nursing Facilities to ensure client satisfaction and retention.
  • Partner with Business Development to support expansion into prospective facilities.
  • Represent the organization in client-facing meetings to reinforce service excellence and partnership.
    Strategic Planning & Growth
  • Identify opportunities for service expansion and partnership development.
  • Lead implementation of new post-acute programs and initiatives.
  • Collaborate with executive leadership on long-term strategic planning.
    Quality Improvement & Compliance
  • Drive quality improvement initiatives to enhance patient outcomes and provider performance.
  • Ensure compliance with all federal, state, and local healthcare regulations.
  • Present and implement new programs and processes that reduce costs while improving quality of care.
  • Participate in organizational strategy, planning, and director-level meetings.
    Program & Strategic Management
  • Support the COO in managing existing and prospective programs and services.
  • Participate in monthly KPI reviews and quarterly leadership meetings.
  • Lead initiatives that align operational execution with broader organizational goals.
    Qualifications
  • Bachelor’s degree in Healthcare Administration, Business, Nursing, or related field
  • Master’s degree (MHA, MBA, MSN, or related field)
  • 5 years of healthcare operations experience
  • 3+ years in post-acute care leadership (SNF, home health, rehab, hospice, or transitional care)
  •  Strong understanding of CMS regulations and value-based care models
  • Demonstrated success in managing multi-site operations and large, diverse teams.
  • Strong knowledge of healthcare regulations, compliance standards, and quality improvement practices.
  • Proven ability to manage budgets, contracts, and financial performance.
  • Excellent communication, leadership, and relationship management skills.
  • Strategic thinker with a results-oriented mindset and ability to drive performance improvement initiatives.
