- Responsibilities
- Clinical Duties:
- Conduct in-home client assessments
- Develop and review care plans
- Coordinate with caregivers and families
- Ensure compliance with state and agency regulations
- Marketing & Outreach:
- Make outbound calls to referral sources (hospitals, rehab centers, assisted living communities, physician offices)
- Set up and attend appointments with potential referral partners
- Build and maintain strong professional relationships
- Represent the agency at networking events
- Online & Social Media Management:
- Manage agency social media platforms (Facebook, LinkedIn, etc.)
- Create and post engaging content
- Respond to online inquiries and leads
- Assist with online reputation management and community engagement
- Requirements
- Active RN license (Virginia)
- Home care or case management experience preferred
- Strong communication and relationship-building skills
- Comfortable making calls and scheduling appointments
- Experience with social media platforms and basic online marketing
- Organized, self-motivated, and professional
