Hospital Liaison (Respiratory Therapist Required)

Position Summary:
We are seeking a dynamic and clinically strong Hospital Liaison to serve as the primary connection between our skilled nursing facilities and hospital partners. This role is ideal for a Respiratory Therapist (RT) with experience in skilled nursing settings who can effectively evaluate patients, build relationships, and facilitate appropriate admissions—particularly for higher-acuity and ventilator-dependent patients.

    Key Responsibilities:
  • Serve as the primary liaison between hospitals, case managers, and our skilled nursing facilities
  • Conduct on-site or remote clinical evaluations of potential admissions, with a focus on respiratory and high-acuity patients
  • Collaborate with hospital discharge planners, physicians, and care teams to coordinate safe and efficient transitions of care
  • Identify appropriate placement opportunities within our facilities based on clinical needs and capabilities
  • Educate hospital partners on our services, including ventilator care and respiratory programs
  • Build and maintain strong referral relationships to support census growth and quality admissions
  • Communicate closely with internal clinical and admissions teams to ensure seamless patient transitions
  • Participate in care planning discussions and provide clinical insight when needed
  • Track referral activity and follow up on pending admissions
    Qualifications:
  • Licensed Respiratory Therapist (RT) required
  • Minimum of 2+ years of experience in a skilled nursing facility (SNF) setting
  • Experience with ventilator management and high-acuity patients strongly preferred
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and relationship-building abilities
  • Ability to work independently while managing multiple priorities
  • Familiarity with hospital discharge processes and post-acute care coordination
  • Valid driver’s license and willingness to travel locally between hospitals and facilities
