[Hiring] RN Navigator, Bone and Joint @American Addiction Centers
Role Description
- This is a remote position. Secondary location is the NEW ASC/MOB Greensboro on Horse Pen Road.
- Utilizes the nursing process to provide patient/family centered care focusing on the physical, emotional, spiritual, cultural, religious, and environmental needs.
- Coordinates the effective and efficient delivery of patient care services through the application of care management principles.
- Acts as a patient advocate throughout the continuum of care including all patient settings and in the community.
- Documents in accordance with departmental reporting standards.
- Implements outreach efforts to establish and maintain positive working relationships with key community agencies.
- Demonstrates the ability to function in a professional setting through active participation in a professional practice model.
- Communicates effectively to promote sharing of information throughout the continuum of care.
- Responsible for ongoing evaluation and quality improvement of the Navigator Program.
- Works closely with community based service providers to identify and address community needs for services and education.
- Provides age/developmentally appropriate patient care in accordance with Age-Specific guidelines.
- Qualifications
- Graduate from an accredited school of nursing, BSN required.
- Minimum two years' experience as a registered nurse in the specialty required.
- Requirements
- Current licensure to practice as Registered Nurse in applicable state.
- Certification as an RN preferred.
- BLS required.
- Requires problem solving, decision making and critical thinking.
- Ability to work in a self-directed environment and with leading teams.
- Demonstrates ability to work in fast-paced environment with multiple interruptions.
- Expertise in MS Office.
- Ability to prepare program reports frequently and efficiently.
- Bilingual preference with fluency in Spanish desirable.
