Care Coordinator

    Duties
  • Schedule and coordinate patient appointments, ensuring efficient use of provider's time
  • Maintain accurate and up-to-date patient records
  • Communicate with patients regarding appointment reminders, follow-up care, and test results
  • Assist with insurance verification and pre-authorization processes
  • Collaborate with healthcare providers to ensure seamless patient care coordination
  • Provide administrative support to the healthcare team as needed
  • 50/50 work from home and out in the field
    Experience
  • Social work
  • Familiarity with medical terminology and procedures
  • Proficient in using electronic medical record (EMR) systems
  • Strong organizational skills and attention to detail
  • Excellent communication and interpersonal skills
  • Ability to multitask and prioritize tasks effectively

Compulsory documentation experience working with Health Homes program and Molina Healthcare's Clinical Care Advance platform
