Physician Assistant: Founding Clinical Hire
About Quel
Quel Health is the first telehealth platform delivering GLP-1 medications for substance use disorders. Built around emerging evidence that GLP-1 receptor agonists reduce craving and consumption across multiple substances, we provide integrated, evidence-based care for patients with alcohol, nicotine, cannabis, opioid, stimulant, and behavioral use disorders.
Our clinical model combines semaglutide and tirzepatide with FDA-approved medications (naltrexone, varenicline, buprenorphine) where indicated, delivered via a subscription telehealth model with active monitoring and dose titration.
The Role
We are hiring our first Advanced Practice Provider, a Physician Assistant who will work directly under the supervision of our Chief Medical Officer to deliver clinical care to patients with substance use disorders across our active states. This is a founding clinical hire. You will help shape clinical protocols, contribute to outcomes research, and grow with the company as we scale.
The role starts at 20 hours per week, with a clear path to full-time as patient volume grows. We expect the 20 hours to be spread across at least three weekdays to support patient continuity and consistent access. Specific days and hours within those weekdays are flexible and can be set by mutual agreement. We anticipate the transition to full-time within 6 to 12 months. The half-time start is intentional: it lets you join the company at the founding stage without leaving a full-time role until you have line of sight to a sustainable full-time position with us.
- What You'll Do
- Conduct initial patient evaluations via telehealth, with asynchronous note review by the supervising physician
- Develop and manage treatment plans across Quel's clinical formulary, including GLP-1 receptor agonists for substance use indications and FDA-approved medications for addiction (naltrexone, varenicline, buprenorphine, and adjunctive medications)
- Provide monthly follow-up visits, dose titration, side effect monitoring, and patient education
- Collaborate with the Chief Medical Officer on clinical protocol refinement based on patient experience and emerging evidence
- Document care in the electronic health record (transitioning to Quel's custom platform during your tenure)
- Participate in case review, clinical quality assurance, and continuous improvement processes
- Maintain compliance with HIPAA, 42 CFR Part 2, and applicable state regulations
- What We're Looking For
- Required: Active, unencumbered Physician Assistant license in Pennsylvania, with willingness to obtain licensure in additional states (typically 8 to 15) over the first six to twelve months. Additional active state licenses are a plus. All licensure costs and processes are managed and fully covered by Quel.
- Required: Active NCCPA certification.
- Required: DEA registration (or willingness to obtain) for prescribing buprenorphine and other Schedule III-V medications as part of patient care.
- Required: Minimum 1 to 2 years of clinical experience in addiction medicine, behavioral health, or a closely related specialty. Direct experience prescribing medications for substance use disorders is strongly preferred.
- Required: Comfort with evidence-based off-label prescribing in an emerging clinical category. You should be excited by, not anxious about, practicing at the frontier of a new therapeutic approach. Familiarity with GLP-1 receptor agonists, the neurobiology of reward pathways, or the emerging literature on GLP-1s in addiction medicine is strongly preferred.
- Required: Telemedicine experience and comfort delivering high-quality clinical care via video visits.
- Strongly preferred: Prior experience prescribing GLP-1 medications in any clinical setting (obesity medicine, endocrinology, primary care, weight management telehealth).
- Preferred: Experience with motivational interviewing, harm reduction principles, or trauma-informed care.
- Preferred: Experience working in a startup or rapidly scaling clinical environment.
- Compensation and Benefits
- Base salary: $60,000 to $70,000 for 20 hours per week ($120,000 to $140,000 full-time equivalent). Compensation scales proportionally upon transition to full-time.
- Equity: Meaningful founding-team equity grant on a standard 4-year vesting schedule with a 1-year cliff, granted at hire at the full founding-team level regardless of part-time start.
- Healthcare: Medical, dental, and vision coverage activate upon transition to full-time.
- Fully covered by Quel: State licensure, DEA registration and renewals, malpractice insurance with tail coverage, credentialing, and collaborating physician arrangements where required.
- Time off: Pro-rated paid time off, plus standard holidays.
Why This Role

If you are a Physician Assistant who has been quietly frustrated that addiction medicine is still treated as a niche specialty rather than as the public health priority it should be, and if you believe that GLP-1s represent a generational shift in how we treat substance use disorders, we want to hear from you.
