  • *Job Summary**

As a Customer Service Representative, you will play a critical role in our Double Qualified Special Needs Plan (DSNP) team, working with federal healthcare and Medicaid members who require specialized care and support. Your primary responsibility will be to provide exceptional customer service, using your clinical knowledge and communication skills to navigate complex healthcare and social needs. You will work closely with our members, healthcare providers, and community organizations to ensure seamless care coordination and optimal health outcomes.

  • *Key Responsibilities**
  • Conduct comprehensive assessments of members' needs and develop personalized care plans to address their healthcare and social determinants.
  • Identify high-risk factors and service needs that may impact member outcomes and collaborate with clinical teams to develop effective care plans.
  • Coordinate and implement care plan activities, monitoring progress and making adjustments as needed.
  • Collaborate with case managers, healthcare providers, and other stakeholders to overcome barriers to care and achieve optimal results.
  • Recognize and report quality of care issues through established channels.
  • Utilize effective communication and persuasion skills to engage members in their care and promote lifestyle changes that improve their health and well-being.
  • Provide education, support, and resources to empower members to make informed decisions about their healthcare and make progress towards their goals.
  • Facilitate member engagement and participation in their care, ensuring they are knowledgeable and empowered to navigate the healthcare system.
  • *Required Qualifications**
  • 3+ years of experience in a healthcare-related field, preferably in case management, social work, or a related field.
  • Strong clinical knowledge and understanding of healthcare systems, policies, and procedures.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work in a fast-paced, dynamic environment with multiple priorities and deadlines.
  • Strong analytical and critical thinking skills, with the ability to make sound judgments and decisions.
  • Proficiency in electronic medical records (EMRs) and other healthcare software systems.
  • *Preferred Qualifications**
  • Certification in case management, such as CRC, CDMS, CRRN, COHN, or CCM.
  • Experience working with geriatric populations, social health, and individuals with disabilities.
  • Bilingual or multilingual skills, with the ability to communicate effectively with diverse populations.
  • Familiarity with evaluation, screening, and care planning processes.
