The ideal candidate will have expertise in form design, proficiency in LaTeX or Adobe Acrobat, and a deep understanding of U.S. business and tax requirements.

• *Key Responsibilities**

As our freelance form designer, you will be responsible for creating a clean, editable PDF template that meets the following requirements:

• **Customer Information**: Include sections for:
+ Legal name+ DBA (Doing Business As)+ Billing address
• **Contact Details**: Include sections for:
+ 2 Accounts Receivable contacts+ 3 key contacts+ Optional Head Service Person
• **Tax and Compliance Info**: Include sections for:

• **Purchasing, Shipping, and Payment Details**: Include sections for:
