The Care Team Associate position at Optum involves providing administrative support and customer service within a community-based medical group. Responsibilities
ย Document all member interactions in compliance with documentation requirements for Model of Care, NCQA, and CMS regulations.

ย Receive and respond to incoming Care Coordination inquiries from various communication channels.

ย Coordinate and assist in monitoring documentation Care Management queues for various referrals.

ย Process Care Management requests and prepare administrative support for clinical staff.
,
ย Conduct inbound and outbound calls for program requirements, including member scheduling and surveys.
,
ย Prepare daily INPT and Post-Acute Census for CM referrals and inform the Care Management Manager of outstanding cases.
,
ย Complete timely data entry of member contact information into software applications.
,
ย Provide clerical and administrative support to clinical staff and managers for special projects and reporting needs.

ย Deliver excellent customer service to all internal and external customers.
,
ย Attend required meetings and participate in ad hoc committees as needed.
,
ย Maintain knowledge of health plan benefits, network, NCQA regulations, and health plan policies.

ย Identify opportunities for process improvement in member care.
,
ย Maintain strict confidentiality and adhere to all department policies.
Requirements
ย High School Diploma / GED or equivalent work experience.
,
ย Must be 18 years or older.
,
ย Medical assistant certification.
,
ย 2+ years of administrative support experience.
,
ย Knowledge of medical terminology.
,
ย Advanced knowledge of Microsoft Office products, including Excel, Word, and Outlook.
,
ย Ability to toggle between multiple IT systems and platforms.
,
ย Ability to work normal business hours of 8:00am - 4:30pm EST, with occasional overtime.
Nice-to-haves
ย 2+ years of experience in a physician's clinic or hospital.

ย 2+ years of experience working in a medical care setting as a receptionist or medical assistant.

ย Ability to prioritize, plan, and handle multiple tasks simultaneously.

ย Ability to work independently with some supervision.
