Clinical Social Worker - Hybrid Opportunity

We are seeking a compassionate and experienced Clinical Social Worker to join our team at Mass General Brigham. As a hybrid opportunity, you will have the flexibility to work onsite 4 days a week and remotely 1 day a week, providing comprehensive care coordination and social work services to our patients.

Key Responsibilities:

• Conduct comprehensive assessments and case management to identify high-risk psychosocial factors and develop personalized care plans


• Provide patient navigation, health promotion, crisis management, and self-management education to empower patients and their families


• Collaborate with interdisciplinary teams to ensure seamless care coordination and effective treatment plans


• Offer short-term counseling, advanced care planning, and liaison services to community resources


• Participate in team consultations and contribute to the development of best practices in social work

Requirements:

• Current LICSW/LCSW in MA


• Master's degree in social work


• Minimum of 2 years of clinical experience, with preference for experience in ambulatory care settings
