About the Role:

As a compassionate and skilled Home Health Registered Nurse (PRN), you will have the opportunity to make a meaningful impact on the lives of patients and their families. Responsibilities:

• Coordinates and oversees all aspects of patients' home care, including developing and implementing care plans


• Collaborates with patient care teams to provide high-quality, patient-centered care


• Educates families on patient condition, needs, and care


• Performs physical assessments, checks vital signs, and manages medications


• Administers injections, IV therapy, or tube feedings, and maintains catheters


• Completes documentation timely and accurately, and communicates patient progress to the clinical manager, physician, and care team


• Supervises LPNs and HHAs, and participates in quality improvement initiatives

Qualifications:

• Current RN license, specific to the state(s) you are assigned to work


• Minimum one year of clinical experience as a registered nurse (RN), or completion of an RN Intern program


• Minimum six months of home health experience


• Current CPR certification


• Valid driver's license, reliable transportation, and liability insurance


• Able to assess patient status and identify requirements relative to age-specific needs


• Proficient in the use of computers, including navigation of computer desktop, email, and online HR services
