As a Case Manager RN, you will play a vital role in ensuring the continuity of care for our patients, leveraging your nursing expertise to drive quality outcomes and improve patient experiences.

Key Responsibilities:

• Utilize your nursing knowledge and skills to facilitate continuum of care, ensuring appropriate resource utilization and patient quality outcomes.


• Prioritize patient care needs, addressing emerging issues and developing personalized care plans in collaboration with patients, families, and caregivers.


• Coordinate treatment plans with the care team, triaging interventions and using protocols and pathways to optimize clinical outcomes.


• Monitor and coach patients using motivational interviewing and behavioral change techniques to maximize self-management.


• Maintain effective communication with physicians, hospitalists, extended care facilities, patients, and families to ensure seamless care transitions.

Requirements:

• Active RN license in current state of residence, with the ability to qualify for additional state licenses as requested.


• 2+ years of nursing experience in a hospital, acute care, or direct care setting.


• Bilingual proficiency in Spanish, Cantonese, or Mandarin.


• Renal, Chronic Kidney Disease, or Dialysis Care experience.


• Computer proficiency, including MS Office and electronic health records.

Preferred Qualifications:

• BSN degree.


• Certified Case Manager (CCM) certification.


• Diabetic educator experience.


• ICU, Cardiology, or Critical Care experience.


• Telephonic case management experience.
