Responsibilities

As a Remote Clinical Case Manager, you will be a vital point of contact for patients, families, providers, and other stakeholders. You will:

• Coordinate patient care across the continuum of care, including referrals, home visits, and community resources.


• Assess patient needs and develop care plans to improve health outcomes and ensure access to benefits.


• Communicate with patients and partners to provide education, support, and advocacy.


• Maintain accurate and timely documentation of case management activities.


• Collaborate with a multidisciplinary team to optimize patient well-being.


• Ensure compliance with all applicable regulations and guidelines.


• Proactively address issues to improve patient outcomes and enhance the overall patient experience.

Qualifications

Required:

• Demonstrated ability to effectively communicate and build rapport with individuals and families.


• Strong organizational and time management skills.


• Ability to work independently and as part of a team in a remote environment.


• Reliable internet access and a dedicated workspace.

Preferred:

• Experience in clinical case management, care coordination, or home medical services.


• Bilingual proficiency in Spanish/English.


• Experience with Federal health insurance populations.


• Valid driver's license and reliable transportation for occasional in-person visits within a designated geographic area.
