Care Coordinator - Bilingual (English/Spanish)

- Assessing transitional care needs
- Coordinating care across the continuum
- Engaging with patient and family to assure care needs are met
- Implementing timely, safe patient discharge plans
- Providing psychosocial assessments
- Offering guidance to patients/families on community-based services.
- Advocating for patient and family involvement in care decisions.
- Documenting assessments and interactions in medical records.
- Requirements:
- Bachelors Social Work or Masters Social Work-Preferred
- BLS - Basic Life Support or BLS-I - Basic Life Support - Instructor
- Minimum 1 year of experience in healthcare in the acute care setting, related field or skilled care environment or community or educational internship in care coordination.
- Excellent written and verbal communication skill.
- Strong assessment, interview, organizational and problem-solving skills.
