Salary: $58,000-$70,000

Position Summary:
The Behavioral Health Veteran Community Care Coordinator assists Illinois Joining Forces in providing comprehensive behavioral health support, care coordination, screening, and resource navigation services to Illinois Service Members, Veterans, and their families. This position works within an inbound/outbound Care Coordination Center and supports individuals through screening, referral, care coordination, and follow-up services. While some individuals may present in crisis, most interactions focus on supportive care coordination, resource navigation, follow-up, and connection to community-based services.
Standard business hours are Monday–Friday with occasional flexibility based on program and client needs.
- Essential Responsibilities:
- Conduct standardized screenings and intake assessments in accordance with organizational policies and procedures.
- Provide clients with information, referrals, advocacy, resource navigation, and care coordination services related to behavioral health and supportive community resources.
- Support individuals experiencing emotional distress, crisis situations, or complex psychosocial barriers while maintaining professionalism and compassion.
- Maintain accurate, timely, and thorough documentation of client interactions, referrals, follow-up activities, and case updates within electronic documentation systems.
- Coordinate with community providers, behavioral health agencies, crisis services, and Veteran-serving organizations to support continuity of care.
- Assist clients in identifying barriers to care and developing practical support strategies and service connections.
- Provide trauma-informed communication and support throughout all client interactions.
- Respond appropriately to crisis situations and follow internal escalation, safety, and documentation procedures.
- Manage multiple client interactions and follow-up activities while maintaining organization, responsiveness, and attention to detail.
- Participate in special projects, program initiatives, outreach efforts, and other organizational activities as assigned.
- Must maintain a private, reliable workspace with stable internet access suitable for handling confidential client communications.
- Qualifications:
- Bachelor’s degree preferred in psychology, social work, counseling, human services, behavioral health, rehabilitation services, or related field. Equivalent education and experience may be considered.
- Minimum of 2 years of experience in behavioral health, crisis services, case management, suicide prevention, care coordination, customer support, or related field preferred.
- Knowledge of military culture, Veteran needs, behavioral health concerns, and community resources strongly preferred.
- Experience working with individuals experiencing PTSD, TBI, substance use concerns, suicidal ideation, and other psychosocial challenges preferred.
- Training or certification in suicide prevention, crisis intervention, trauma-informed care, or related behavioral health support strongly preferred.
- Ability to remain calm, professional, organized, and effective in potentially high-stress or fast-paced situations.
- Strong communication, empathy, active listening, documentation, and problem-solving skills required.
- Experience using Microsoft Office, electronic documentation systems, CRM platforms, or case management systems required; Salesforce experience preferred.
- Ability to work independently in a remote environment while maintaining communication, accountability, and collaboration with team members.
- Familiarity with confidentiality standards, professional boundaries, and sensitive client information handling preferred.
This position provides supportive care coordination, resource navigation, and behavioral health support services and does not provide clinical therapy or medical treatment.
