**Experienced Intake Customer Service Representative – Home Health Operations**

As an Intake Customer Service Representative, you'll be responsible for providing exceptional customer service to potential patients and referral sources, evaluating patient suitability for home health care, and coordinating new patient onboarding with our marketing team. You'll work closely with our Intake Manager to ensure smooth communication and efficient data entry into our computer system. - *Key Responsibilities**
- Answer phone calls from potential home health patients and referral sources, such as hospital case managers and physician's offices
- Describe arenaflex's home health service options to patients and referral sources
- Evaluate and determine whether the patient is suitable for home health care over the phone
- Verify that the patient's payer source is in the network with arenaflex
- Send and receive faxes with vital supporting documentation, signed orders, etc.
- Follow up with physicians, clinics, long-term care facilities, hospitals, etc.
- Coordinate new patient onboarding with our marketing team to ensure satisfactory customer service to the referral sources and our patients
- Input new patient data into the computer system and assist the RN with tracking and data reporting
- Perform other duties as assigned by the Intake Manager

- *Requirements**
To be successful in this role, you'll need:
- Three to five professional references that can attest to your skills and experience in a medical office setting
- Recent experience working in a clinic or other professional medical office setting, with a minimum of one year of experience
- A solid history of excellent customer service in an office setting
- Excellent computer skills and a working knowledge of office machinery (computers, multi-line phones, fax machines, email, etc.)
- Knowledge of medical terminology is a must-have skill
- Experience with patient intake and processing new patient referrals is preferred
- Experience in a Home Health office is a plus but not required
