We are a durable medical equipment (DME) provider operating in Ohio, Michigan, Pennsylvania, and South Carolina. Our company develops specialized pediatric sleep safety solutions and is preparing to apply for funding through the Rural Health Transformation Program (RHTP) and similar state grant programs. South Carolina’s RHTP offers grants for projects that modernize rural healthcare infrastructure, expand workforce capacity, and improve health outcomes. We intend to submit a proposal to support a Pediatric Sleep Safety & Assistive Technology Trial Program—a program that provides temporary inflatable safety beds, a caregiver tracking app, and provider oversight to improve safety outcomes for children aged 2–6 with developmental or medical conditions. We also plan to explore equivalent funding opportunities in Ohio, Michigan, and Pennsylvania as those states implement their own RHTP or related programs.

Scope of Work

We seek an experienced grant writer to:

Research program requirements for South Carolina’s RHTP and similar programs in Ohio, Michigan, and Pennsylvania. Identify the best initiative or funding stream for our project in each state, analyze eligibility criteria, and outline application requirements and evaluation priorities.

Develop compelling grant narratives that describe our Pediatric Sleep Safety initiative, demonstrate how it meets program goals (e.g., improving rural healthcare access, workforce development, technology innovation), and highlight anticipated outcomes such as reduced injury risk and healthcare cost savings.

Prepare detailed budgets and justifications aligned with allowable costs under each state’s program. Work with our team to refine cost estimates for personnel, equipment, technology development, travel, and evaluation.

Assemble and format complete application packages according to each state’s submission guidelines, including required forms, letters of support, and attachments. Coordinate internal and external stakeholders to gather necessary documentation and feedback.

Support additional state grant applications by adapting our program narrative for Ohio, Michigan, and Pennsylvania, and preparing submissions as new opportunities become available.

Deliverables

A strategy memo summarizing applicable grant opportunities in SC, OH, MI, and PA and recommending the most appropriate application paths.

A draft grant narrative and budget for the South Carolina RHTP submission.

Completed application package for South Carolina’s RHTP (ready several days before the official deadline) and draft materials adaptable for other states.

Ongoing support to identify and respond to subsequent grant opportunities in Ohio, Michigan, and Pennsylvania.

Qualifications

Proven experience winning federal or state health‑related grants, especially for programs involving rural health, pediatric care, or healthcare infrastructure.

Strong research skills to interpret complex program guidelines across multiple states and translate them into actionable proposals.

Excellent writing and editing ability to craft persuasive narratives.

Knowledge of durable medical equipment and Medicaid/Medicare programs is an asset.

Ability to manage multiple deadlines and coordinate with diverse stakeholders.

Compensation

This is a freelance engagement; compensation is negotiable based on experience. Please provide your proposed rate and estimate the hours required.

How to Apply

Please submit a brief proposal outlining your experience with similar grants and your approach to managing multi‑state applications. Include a writing sample if possible. Do not include any contact information (email addresses, phone numbers, or links to third‑party messaging services) in your response; all communication will be conducted through Upwork.
