Role Description

    This is a remote position. Secondary location is the NEW ASC/MOB Greensboro on Horse Pen Road.
  • Utilizes the nursing process to provide patient/family centered care focusing on the physical, emotional, spiritual, cultural, religious, and environmental needs.
  • Coordinates the effective and efficient delivery of patient care services through the application of care management principles.
  • Acts as a patient advocate throughout the continuum of care including all patient settings and in the community.
  • Documents in accordance with departmental reporting standards.
  • Implements outreach efforts to establish and maintain positive working relationships with key community agencies.
  • Demonstrates the ability to function in a professional setting through active participation in a professional practice model.
  • Communicates effectively to promote sharing of information throughout the continuum of care.
  • Responsible for ongoing evaluation and quality improvement of the Navigator Program.
  • Works closely with community based service providers to identify and address community needs for services and education.
  • Provides age/developmentally appropriate patient care in accordance with Age-Specific guidelines.
    Qualifications
  • Graduate from an accredited school of nursing, BSN required.
  • Minimum two years' experience as a registered nurse in the specialty required.
    Requirements
  • Current licensure to practice as Registered Nurse in applicable state.
  • Certification as an RN preferred.
  • BLS required.
  • Requires problem solving, decision making and critical thinking.
  • Ability to work in a self-directed environment and with leading teams.
  • Demonstrates ability to work in fast-paced environment with multiple interruptions.
  • Expertise in MS Office.
  • Ability to prepare program reports frequently and efficiently.
  • Bilingual preference with fluency in Spanish desirable.
