Hybrid Telehealth Nurse Practitioner needed in East Stroudsburg, PA!
An outstanding employee health center is seeking a Nurse Practitioner for a hybrid telehealth role in East Stroudsburg, PA. This position offers excellent work-life balance with no on-call duties and a weekday schedule! Also, this is a perm position with great compensation and competitive pay!
Job Responsibilities: - APP's serve multiple serious conditions like CKD and/or ESRD/ESKD
- Conducts assessments including annual wellness exams
- Counsels and educates patients and their families about programs
- Responsible for coordination of care with primary care providers
- Part of the Integrated care team working with RN and Social Worker
- Prescribe medications, order tests, collaborate with Monogram physician
- Follow up and evaluate patient's status Qualifications: - NP, FNP, AGNP, ACAGNP with Active PA license
- Board Certified- ANCC or AANP
- 2 or more years of direct patient care
- DEA, BLS Working Hours: Mon-Fri (8a-5p includes travel time for in-home visits)
