About the Role:
We are seeking a compassionate and skilled Home Care Registered Nurse (RN) to join our team, providing acute care to patients in the comfort of their own homes. Responsibilities:
- Conduct nursing assessments, medication reconciliation, safety assessments, and implement standardized protocols
- Administer prescribed medications, obtain blood samples, insert foley catheters, and provide therapeutic interventions
- Educate patients on managing their illness, prescribed medications, nursing interventions, and care plans
- Identify and act on changes in patients' conditions, evaluating and revising care plans as needed
- Document all assessments, interventions, and care coordination details
- Participate in regular in-service or special case orientations, on-call rotation, and other duties as assigned

Requirements:
- Associate of Science in nursing
- Current RN license, specific to the state(s) you are assigned to work
- One year of adult acute inpatient medical, or medical/surgical care experience within the last five years
- Current CPR certification
- Valid driver's license and reliable transportation
Preferred Qualifications:
- Bachelor of Science in nursing
- Experience providing RN services in the home, community, or clinic
- Adult medical critical care experience
