Responsibilities:
As an LPN Home Health professional, you will be responsible for:
- Verifying the plan of care with the RN prior to each visit
- Collaborating with physicians and RNs to ensure patient needs are being met
- Teaching patients and caregivers self-care techniques
- Checking vital signs and reviewing medications
- Performing specialized procedures ordered by physicians
- Preparing equipment and materials for treatments using aseptic technique
- Completing documentation in a timely and accurate manner
Qualifications:
To be considered for this role, you must have:
- One year of clinical LPN experience
- Current LPN license, specific to the state(s) you are assigned to work
- Current CPR certification
- Valid driver's license, reliable transportation, and liability insurance
