Physical Therapist - Physical Therapy - Per Diem
We welcome you to visit our beautiful, 2,800 sq. foot treatment facility, which
features a 19x20 turf room for athletic performance! The rehab facility is
embedded into the Surgical Specialties building, including Orthopedics,
Neurosurgery, Interventional Pain, Cardiology, Pediatrics, and Occupational
Medicine.
Position Summary:
The incumbent shall be responsible for providing a full range of physical
therapy services including assessment, treatment planning and therapeutic
interventions in an interdisciplinary environment consistent with the positionâs
qualifications, professional practices and ethical standards. The incumbent
shall also demonstrate accountability for and contribution to program
development, quality improvement, problem solving, and productivity enhancement
in an interdisciplinary setting.
Education, License & Cert:
The incumbent shall, at a minimum, be a graduate of an accredited fourâyear
Bachelorsâ degree program (or foreign equivalent). Current licensure in the
state of practice is required. Current BLS Certification mandated.
Experience:
Education/experience in working with patients to assist in rehabilitation of
disabled or injured individuals throughout their lifespan.
- Essential Functions:
- Communicates and demonstrate a professional image/attitude for patients, families, clients, coworkers, and others.
- Conduct initial assessments, chooses the appropriate assessment tools, chooses appropriate adaptive equipment/materials used in the assessment basedon the age and the development level of the patient, patient assessment iscompleted and in the medical record as outlined in policy and procedure. Datais documented in the appropriate areas.
- Develop individualized treatment plans, short and long term goals are written objectively and are measurable, specifically related to a functional deficitif ongoing treatment planned within a specific time frame. Interventions areselected based on assessment results and are appropriate for age, cognitiveabilities and/or development level of the patient and may includeadministering medication as ordered.
- Evaluate patient response to treatment. Patient response to the treatment plan is evaluated as evidenced by patient achievement of goals and/or goalrevision, the treatment plan is updated as needed. Information is providedto patient/family regarding current status, program planning and dischargeplans throughout the rehabilitation program.
- Document the course of patient care including progress made to justify need for skilled intervention versus lack of progress and further clinicalrecommendation communicated with referring providers. Interventions andeffects are documented daily according to policy and procedure. Dailyprogress notes are completed according to policy and procedure. Progressnotes in outpatient are to be completed at least every 10 visits and/or perstate practice act. Appropriate capture of charges related to treatmentprovided. Discharge summaries are completed according to policy andprocedures.
- Provide patient and family education on tasks and resources that will assist with continued functional independence. Integrates family members into thetreatment, educational and discharge planning process for a safe transitionhome, and to ensure continued care needs are met. Effectiveness of educationmeasured by peer input, family verbal feedback and patient satisfactionsurveys and quality metrics. Educational methods are adapted to meet patientand family education needs. Participates with multidisciplinary team, asneeded, in all aspects of discharge planning including determination ofappropriate next level of care.
- Participating in continuing education/professional development activities. Attends/presents at departmental inâservice sessions as scheduled. Attendsexternal professional growth activities as scheduled and communicatesinformation to the team.
- Adhere to policies and practices of applicable federal and state guidelines, professional organizations, entity facilities and entity specific policies.Is familiar with and acts according to contents of applicable professionalcode, employee/handbook, unit Policy & Procedure and organizationalpolicies.
- Meets established organizational productivity standards within a fiscally responsible manner.

The contracted rate of pay for this per diem position is $68.00/hr
