Regional Care Manager - based near Contra Costa County, CA (looking for MSW/BSW, RN/LVN or PT/OT/ST)
The Regional Care Manager for Contra Costa County and surrounding counties, will serve ALS patients and families in a variety of areas including psychosocial support, information and referral, and advocacy surrounding health issues and benefit questions. The Regional Care Manager will guide and monitor patients and families through the progression of the illness, refer patients and families to community services, facilitate monthly support groups, serve as liaisons to ALS Clinics, and assist with the implementation of treatment recommendations made by clinic staff. Under the direction of Organizational leadership, the Care Manager will represent The ALS Network, as needed, in statewide initiatives related to mission priorities of care services, advocacy, and research.
Requirements: Currently residing in the state of California, based in or near Contra Costa County, and familiar with local resources. A bachelor's degree in a healthcare discipline, or at least 5-7 years of experience working with individuals living with significant disability or impairment. Excellent attention to detail, sensitivity to disability issues, timely data tracking and reporting. Must have a current drivers license and the ability to get around assigned areas.
Direct service to ALS patients, families and caregivers can be provided by phone, email, virtual meeting platforms, and in-person visits to patientsย homes.
Major Responsibilities
A. Monitor and assist ALS patients, families and caregivers in alignment with organizational protocols and productivity expectations.
Assess psychosocial needs of patients, families and caregivers, and provide appropriate counsel, information, and referrals including assistance with:
ยท Adjustment to diagnosis
ยท Coping with the challenges of the disease
ยท Managing the transitions of the illness
ยท Ongoing guidance and psychosocial support to patients and families
ยท Work closely with Director of Care Services for approval of equipment and direct assistance expenditures on behalf of clients
ยท Other administrative duties as assigned.
B. Provide information, referrals and access to equipment and services ยท Provide referrals to ALS Certified Centers and Clinics ยท Inform patients and families about ALS Network services
ยท Serve as an advocate for patients and families
ยท Facilitate or arrange/oversee support groups
ยท Provide information and referral to health, community and financial services
ยท Provide referrals to local home health agencies and other vendors for services such as respiratory, social work or occupational therapy
ยท Facilitate equipment acquisition to assist with mobility, safety, respiration and communication
C. Serve as an ALS Network and client liaison to the healthcare, research, and advocacy-related community
ยท Represent the ALS Network at ALS clinics, providing information regarding ALS Network services & events and assisting people with ALS implementing clinic recommendations at home and in the community
ยท Establish and foster strategic community and professional partnerships and collaborations, including designing, planning, and delivering relevant trainings
ยท Maintain relationships with local neurologists and other providers through outreach activities
ยท In collaboration with Organization Leadership, represent The ALS Network initiatives connected to local research and advocacy in areas assigned.
