They are seeking a full-time Care Coordinator to support their expansion into Chronic Care Management, focusing on providing relationship-based support for adults managing chronic conditions.

    Responsibilities
  • Manage a defined caseload of enrolled CCM (chronic care management) members
  • Conduct proactive outreach to support ongoing engagement and care coordination
  • Identify and address social determinants of health, including food access, housing resources, transportation, and community supports
  • Coordinate referrals to community-based organizations and social service providers
  • Track referral progress and support members in navigating next steps
  • Partner closely with the RN and NP to surface barriers impacting care or engagement
  • Support appointment scheduling, reminders, and follow-up coordination as needed
  • Maintain accurate, timely documentation of member interactions and coordination activities
  • Meet outreach, documentation, and engagement expectations aligned with CCM requirements
  • Collaborate closely with the care team to deliver team-based, whole-person support
    Skills
  • Minimum of 1 year of experience in care coordination, community health work, or a related member-facing support role
  • Bilingual - Spanish and English fluency required
  • At least 1 year of experience working with a Medicare/Medicaid population
  • Experience working in healthcare support call center settings
  • Experience supporting individuals with chronic conditions and ongoing care needs
  • Strong understanding of social determinants of health and community-based resources
  • Comfort working in structured, metrics-driven programs
  • Strong communication skills and ability to build rapport with diverse populations
  • Ability to work independently in a fully remote environment
  • Proficiency with documentation systems, CRMs, or EHRs
  • Reliable internet connection and access to a HIPAA-compliant workspace
