We are seeking a dedicated and professional Remote Healthcare Consultant to join our team, overseeing case management activities for enrollees in our Long Term Care/Comprehensive Program. As a Remote Healthcare Consultant, you will play a pivotal role in ensuring favorable healthcare outcomes through effective care coordination, support, and education.


Key Responsibilities:



  • Employ critical thinking and sound judgment to aid in the case management process, utilizing care management tools and data insights to drive informed decisions.

  • Perform thorough evaluations of members, coordinating and executing assigned care plan activities, and vigilantly monitoring progress to achieve optimal outcomes.

  • Facilitate multidisciplinary reviews, identify and elevate quality of care concerns, and leverage negotiation and motivational interviewing techniques to secure appropriate services and encourage lifestyle modifications.

  • Provide guidance, information, and support to empower members in making autonomous health decisions, and assist members in actively collaborating with their healthcare providers in decision-making.

  • Adhere to case management and quality management processes in alignment with regulatory and accreditation standards, ensuring exceptional patient care and outcomes.


Required Skills and Qualifications:



  • Bilingual proficiency in Spanish and English, with strong communication skills, both verbal and written.

  • Proficiency in Microsoft Office, particularly Excel, with exceptional critical thinking and judgment abilities.

  • Capable of multitasking and prioritizing effectively in a dynamic environment, with skills in negotiation and motivational interviewing.

  • Bachelor's degree in Social Work or a closely related field, with demonstrated case management experience, ideally with a background in long-term care.

  • Willingness to travel approximately 75% within the region to engage members in their homes, assisted living facilities, and nursing homes, with residency in Miami-Dade County, FL.
