Position Overview


As a Remote Patient Advocate, you will leverage your pharmacy expertise and exceptional customer service skills to provide support to Medicare recipients within a Government healthcare ACO plan. You'll work remotely, communicating directly with beneficiaries via telephone to explore options for medication cost assistance. You will collaborate with a multidisciplinary team to ensure comprehensive support for our members.

Key Responsibilities:



  • Conduct remote phone consultations with Medicare recipients to assess their medication needs and identify potential cost-saving resources.

  • Gather information from staff and beneficiaries to provide resources, assist with applications, and address medication cost concerns.

  • Contact pharmacies and suppliers as needed to gather information.

  • Maintain and update a database of accessible cost-saving resources.

  • Collaborate with pharmacists, nurses, social workers, and community health workers to meet the holistic needs of beneficiaries.

  • Adapt to evolving workflows and priorities.

Qualifications


Required:



  • Licensed and registered pharmacist in good standing.

  • Minimum of 3 years of experience as a pharmacist.

  • Excellent verbal and written communication skills.

  • Proficiency in basic computer skills, including the ability to learn new systems (e.g., Epic).

  • Ability to work independently and manage workload efficiently.

  • Ability to adapt to changing workflows and priorities.

  • Reliable high-speed internet access and a confidential home workspace.


Preferred:



  • National Clinical Pharmacy Certification (PTCB or ECPT).

  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook).

  • Experience with Epic or other EMR systems.


Education:


High school diploma or equivalent required.
