As a Remote RN Clinical Care Coordinator, you will play a vital role in managing the care of members with complex medical and behavioral needs, promoting improved health outcomes, and enhancing the overall quality of care.


Key Responsibilities:



  • Conduct comprehensive needs assessments to identify members' medical, behavioral, and socioeconomic needs

  • Develop and implement personalized care plans, leveraging evidence-based guidelines to address chronic health conditions, health promotion, and wellness

  • Collaborate with internal care teams, providers, and community resources to ensure seamless care coordination and support member self-management

  • Provide education, coaching, and advocacy to empower members and their families, ensuring their needs and choices are fully represented and supported

  • Proactively manage care transitions, including discharge planning and post-acute care coordination


What You'll Need:



  • Undergraduate degree or equivalent experience

  • Current, unrestricted independent licensure as a Registered Nurse in Tennessee

  • 2+ years of clinical experience

  • 1+ years of experience with MS Office, including Word, Excel, and Outlook

  • Reliable transportation and ability to travel up to 75% to meet with members and providers throughout Putnam, Jackson, and White Counties


Preferred Qualifications:



  • BSN, Master's Degree, or higher in a clinical field

  • CCM certification

  • 1+ years of community case management experience coordinating care for individuals with complex needs

  • Experience working in team-based care, managed care, or a similar setting
