Key Responsibilities:



  • Evaluate members' care needs and develop personalized care plans to achieve optimal health and well-being

  • Coordinate and implement care plan activities, monitor member care needs, and identify high-risk factors and service needs

  • Collaborate with case managers, supervisors, Medical Directors, and other health programs to overcome barriers and achieve optimal outcomes

  • Utilize negotiation and influencing skills to secure appropriate options and services necessary to meet members' benefits and healthcare needs

  • Provide coaching, information, and support to empower members to make informed healthcare decisions and promote lifestyle and behavior changes

Requirements:



  • Minimum 6 months of experience in Foster Care and juvenile justice systems, Adoption Assistance, Behavioral Health Services, Trauma-informed Care, ACEs, Crisis Intervention services, or evidence-based practices applicable to the Kentucky SKY populations

  • Minimum 1 year of experience in behavioral health, social services, social work, psychology, sociology, marriage and family therapy, or counseling

  • Minimum 1 year of experience with MS Office Suite applications, including Word and Excel

  • Willingness to travel 80-90% of the time to meet members face-to-face in the assigned region

  • Residency in the Northern Bluegrass region of Kentucky (Boone, Bourbon, Campbell, Carroll, Gallatin, Grant, Harrison, Kenton, Nichols, Owen, Pendleton, and Scott counties)

  • Bachelor's degree or non-licensed master's level clinician in behavioral health or human services field
