Position Summary



We are looking for a Pediatric Health Coach, bilingual in Spanish and English, to work directly with children and families within the South County community to improve health outcomes under the Healthy Together program. The Pediatric Health Coach will provide and utilize evidence-based strategies around lifestyle management, behavior-change, and disease prevention within pediatric populations. The Pediatric Health Coach is responsible for identifying lifestyle modification opportunities for children and their families, establishing goals and action plans, and conducting follow-up to support and inspire sustainable behavior change. The Pediatric Health Coach will work closely with pediatric patients and family members to promote wellness through one-on-one visits and group health education classes. The Pediatric Health Coach will also refer and connect program participants to local community resources where appropriate. NOTE: This is a two-year grant-funded position. Continuation of this position is subject to ongoing grant funding. This position also requires driving. In addition to a valid CA driver's license, the following are required: Automobile insurance as required by the State of California, an acceptable driving record of three points or less and no DUI within the last 10 years, a current DMV printout, completed pull-form consent for participation in the DMV pull-notice program, and an automobile available for daily work



Responsibilities



Provide health coaching to children and families to promote healthy lifestyle behaviors, improved health outcomes, and facilitate self-management decision support. Act as a liaison and advocate between the patient, the patient's support network, treating physician(s), ancillary providers, and community resources, while utilizing a multi-disciplinary approach and following protocols to assess, identify, plan, goal set and measure optimal clinical outcomes. Employ behavior change and motivational interviewing skills to assess readiness, address barriers to change, establish health goal setting, address short and/or long-term needs, engage member's plans for change while following standard policy and procedure, clinical guidelines, and evidenced-based criteria. Work collaboratively with fellow coaches and internal and external stakeholders including, but not limited to; physicians and their office staff, community members, and CBOs or agencies to identify individuals who require additional wellness and prevention services and connect patients to local services as appropriate. Support patient outreach and enrollment efforts and the development, implementation, and evaluation of Aspire Health wellness programs and population health needs. May include, but not limited to; program promotion, biometric screenings, content development, providing health education lectures/class and health campaigns.

Required Qualifications:



Bilingual English/Spanish



Excellent oral and written communication skills



Ability to effectively establish rapport with patients, providers, and stakeholders in person and via telephone and/or online modalities



Associate of Science, equivalent or higher, in health related field (i.e. health, wellness, nutrition, exercise science, etc.) from an accredited college or university



Minimum of 1 year of experience providing impactful health promotion or disease prevention programs/services/techniques



Ability to tailor information at the appropriate level of literacy and demonstrate cultural competence



Knowledge of managed care concepts, health promotion strategies, motivational interviewing, transtheoretical model and other behavior change tools



Strong commitment to teamwork and excellence and follow leadership directives



Ability to travel with reliable transportation to Monterey, CA and across South County locations



Ability to thrive in a continually evolving environment and take ownership of their work



Strong organizational and prioritization skills



Proficiency in Windows-based database applications (Outlook, Word, Excel)



Preferred Qualifications



Bachelor's degree in a health related field (i.e. health promotion, public health, nutrition, kinesiology, etc.)



Experience working with pediatric populations



Resident of South County



Health Coach, or related certification



ACSM, CHES, or related certification



Experience working in a healthcare setting and/or documenting in electronic medical records



Licensure:



BLS certification prior to employment required



Valid driver's license and personal transportation required



Salary Range: $62,400-$74,800
