Job Description:


The successful candidate will be responsible for:



  • Leading and supervising a team of care management staff, providing guidance, support, and performance feedback

  • Overseeing caseloads, ensuring timely communication with members, providers, and community-based organizations

  • Providing training, resolving staff issues, and identifying areas for program improvement

  • Collaborating with management to perform annual reviews, disciplinary actions, and goal setting

  • Establishing and sustaining relationships with community systems to facilitate linkage, referral, advocacy, and follow-up

  • Assisting with member concerns and grievances, and participating in internal and external audits


Skills and Qualifications:


To be successful in this role, you will need:



  • Excellent communication, negotiation, and interpersonal skills

  • Ability to work effectively in a cross-functional team environment

  • Strong computer literacy and skills

  • Self-motivation to seek knowledge and improve personal and professional performance

  • Regular attendance and a strong work ethic


Preferred qualifications:



  • Master's degree

  • Spanish language proficiency (strongly preferred)

Work Schedule:
